Application
When a prospective resident is interested in Youville House, an Application, as well as a completed Medical Information Form is submitted to Youville House by the primary care or attending physician.

Once the apartment of choice becomes available, the prospective resident signs the "Community Fee Agreement" and submits the community fee. This fee enables our Wellness Director to complete an in-depth assessment in order to determine whether Youville House can meet the care needs of the prospective resident.* Once approved, the Residency Agreement is given to the prospective resident for review.

Wellness Assessment
The Wellness Nurse meets with the prospective resident and family to complete the assessment.

The Wellness Nurse begins to develop the resident's individualized service plan.

Move-In Paperwork
The Residency Agreement is signed.

Fees due at the signing of the Residency Agreement include:

1. A check made out to Youville House for the pro-rated fee for first month's rent.
2. A check equal to one month's rent. This check serves as the security deposit, which is returned with accrued interest at the termination of the Residency Agreement if no damage has been done to the apartment.

Move-In
Youville House assists with the coordination of the resident's move-in and supports the resident throughout the transition from independent to assisted living. The special attention given by staff to each new resident plays a major role in their integration into the community.
